To me, these are caused by bacteria attack. I observed that weaker strains like opaque whites for eg get it more easily. Cure is simple, clean water + bacteriacide[eg yellow powder].

Also possible: chlorine burn (tap water not well aged/treated). In this case, I've noticed yellow powder to actually aggravate matters.
